Certificate of Release or Discharge from Active Duty

VA Community Care Referral Authorization
Authorization document from the VA allowing you to receive care from a community (non-VA) provider. Issued when the VA cannot provide timely care or when you meet eligibility criteria under the MISSION Act.
Application for Disability Compensation
The primary form to file a VA disability compensation claim for service-connected conditions. Can also be used to increase an existing rating.
Intent to File a Claim
Establishes an effective date for your future claim, protecting your potential back-pay. Gives you up to one year to submit the full application.
Statement in Support of Claim
A general-purpose statement form to provide additional evidence, explanations, or context for any VA claim. Commonly used for buddy statements.
Statement in Support of Claim for PTSD
Provide details about traumatic events related to your PTSD claim, including dates, locations, and unit assignments during the stressor events.
Statement in Support of Claim for PTSD (Personal Assault)
Specifically for PTSD claims related to personal assault, military sexual trauma (MST), or harassment. Allows alternative forms of evidence.
Supplemental Claim
Submit new and relevant evidence to have a previously denied claim reconsidered. Part of the Appeals Modernization Act decision review process.
Higher-Level Review
Request a senior reviewer to re-examine your claim based on existing evidence. No new evidence can be submitted, but errors can be corrected.
Board of Veterans Appeals (Board Appeal)
Appeal a VA decision to the Board of Veterans' Appeals. Choose from direct review, evidence submission, or a hearing with a Veterans Law Judge.
Application for Increased Compensation Based on Unemployability (TDIU)
Apply for Total Disability based on Individual Unemployability when service-connected disabilities prevent you from maintaining gainful employment.
Request for Employment Information (Employer Verification)
Sent to employers to verify your employment history and income. Used to support TDIU claims and pension applications.
Examination for Housebound Status or Aid and Attendance
Medical examination form to establish eligibility for Aid and Attendance or Housebound benefits. Must be completed by a licensed physician.
Add or Remove Dependents
Add a spouse, child, or dependent parent to your VA benefits, or remove a dependent after a life change. Affects compensation rates for rated veterans.
VA Rating Decision Letter
Official letter from the VA documenting your disability rating percentage and effective date. Not a form you fill out but a document you receive after a claim decision.
VA Benefits Summary Letter (Proof of Benefits)
Official letter confirming your VA benefits status, disability rating, and monthly payment amount. Download instantly from VA.gov. Often required for state benefits.
Nexus Letter (Medical Opinion)
A letter from a qualified medical professional linking your current condition to your military service. Critical evidence for disability claims. Not a VA form; obtained from your doctor.
Buddy Statement (Lay Evidence)
A written statement from a fellow service member, friend, or family member supporting your claim by describing your condition or the events that caused it. Use VA Form 21-4138 or a personal letter.
Disability Benefits Questionnaire
Standardized medical evaluation forms used by VA and private physicians to document the severity of specific conditions. Available for most ratable conditions.

Compensation and Pension Examination Report
Medical examination report completed by a VA or contract examiner evaluating the severity and service connection of your claimed conditions. You do not fill this out but should request a copy for your records.
Service Treatment Records (STRs)
Your complete military medical file documenting all treatment, injuries, and health conditions during service. Critical evidence for disability claims. Request through the National Archives or your branch's records office.
Line of Duty (LOD) Determination
Official determination that an injury or illness occurred in the line of duty. Especially important for Guard and Reserve members to establish that a condition happened during a qualifying period of service.

Application for 10-Point Veterans' Preference
Claim veterans' preference points for federal civil service hiring. 10-point preference is for disabled veterans, Purple Heart recipients, and certain spouses and parents.

Specially Adapted Housing (SAH) / Special Housing Adaptation (SHA) Grant

Temporary Residence Adaptation (TRA) Grant
Apply for a grant to adapt a family member's home where you are temporarily residing. Available to veterans eligible for SAH or SHA grants.
Servicemembers' Group Life Insurance Election and Certificate
Elect, reduce, or decline SGLI coverage. Also used to designate beneficiaries for your SGLI policy. Active duty members are automatically enrolled.
Claim for One Sum Payment (Insurance Claim)
File a claim for proceeds from a VA life insurance policy after the death of the insured veteran. Submitted by the designated beneficiary.

VGLI Application (Veterans' Group Life Insurance)
Convert your SGLI coverage to Veterans' Group Life Insurance within 1 year and 120 days of separation. Provides renewable term life insurance up to $500,000 without a health exam if applied within 240 days.
