- Register to become an approved State Vendor for Kentucky and wait to receive your vendor ID number starting with KY or KS.
- Verify the veteran's service by DD-214 or discharge form before requesting a stipend.
- Use the new KDVA Form 01-20, Veterans Service Organization Request for Honors Stipend, and complete a separate form for each honor performed.
- Obtain signatures from the Funeral Home Director and the Guard Commander or adjutant on KDVA Form 01-20.
- List the post name and number that is registered as the vendor on KDVA Form 01-20.
- Submit the completed form within 90 days after the burial service by email, fax, or mail to KDVA Accounts Payable.
- Do not submit DD-214s with the request form and do not give the request to the cemetery.
