Eligibility

You may be eligible for a Presidential Memorial Certificate if you meet both of these requirements. The Veteran or Reservist is eligible for burial in a national cemetery, and You’re the next of kin, family member, or close friend of the Veteran or Reservist (or an authorized service representative for a family member or friend of the Veteran or Reservist)

How to apply

1. 1If the Veteran was buried in a national cemetery, expect VA to automatically present a Presidential Memorial Certificate to the next of kin at the burial.
2. 2If the Veteran was eligible for burial in a national cemetery but was buried in a private cemetery, complete Presidential Memorial Certificate Request Form (VA Form 40-0247).
3. 3Apply online, or complete VA Form 40-0247 and choose mail, upload, in-person, or fax submission.
4. 4If mailing, send the completed form with copies of the Veteran’s death certificate and DD214 or other discharge documents to the NCA FP Evidence Intake Center, PO Box 5237, Janesville, WI 53547.
5. 5If uploading, submit the completed application and supporting documents using the QuickSubmit tool.
6. 6If applying in person, submit your application at any VA regional office.
7. 7Do not send original documents because VA says it can’t return them.
8. 8If more than 4 months pass without receiving the certificate, call VA to check the status and do not send a second application unless asked.
